About the role
The Community Health Nurse (Rural/Remote) will provide high level clinical nursing and care coordination support to GPs and other staff to ensure the smooth operation of the practices. This role is responsible for providing primary health care nursing support to aid the day-to-day operations of the cohealth Bicheno site.
Your duties include:
- Initial assessment, triage and management of urgent care presentations at the site
- Implementing and conducting preventative and aged-based health assessments, care planning, and case conferencing
- Supporting GPs in implementing treatmentsand client management plans
- Provide courteous advice and assistance to colleagues, clients and visitors of the site in accordance with the cohealth Code of Conduct

Our ideal candidate:
- Current Registration with AHPRA as a Registered Nurse
- Nurse Immuniser authorisation (or willingness to obtain within 6 months of commencement)
- Advanced Life Support (ALS1) certification (or willingness to obtain within 6 months of commencement)
- Tasmanian Drivers Licence
